Re: NJDA Division of Animal Health to begin charging for forms/supplies

 

The dire economic situation facing the State of New Jersey requires the Division of Animal Health to begin charging for some forms, supplies, and shipping costs. This change will become effective Monday, January 26, 2009.

 

Costs will be as follows:

  • Coggins (EIA) forms = $7.00 per envelope containing 1 to 3 packets for veterinarians using the NJDA laboratory and $15.00 per envelope containing 1 to 3 packets for veterinarians who do not use the NJDA
    laboratory.

  • CVI forms = $3.50 per booklet (containing 25 sets of quadruplicate forms) + $7.00 per envelope containing 1 to 6 booklets

  • Brucellosis Test forms = $7.00 per envelope containing 1 to 3 packets

  • Brucellosis Vaccination forms = $7.00 per envelope containing 1 packet

  • Tuberculosis Test forms = $7.00 per envelope containing 1 to 3 packets

  • Eartags = $7.00 per envelope containing 1 to 3 boxes

  • Pullorum Antigen = $7.00 per vial + $20.00 FedEx shipping charge

  • Brucella Vaccine = $3.50 per 5 doses + $20.00 FedEx shipping charge

Veterinarians who do not regularly use the services of the NJDA Animal Health Diagnostic Laboratory will be charged the following for Styrofoam mailers:

  • Small Styrofoam Mailers (holds 5 tubes) - 1 box of 24 mailers = $65.00 (includes $5.00 shipping charge)

  • Large Styrofoam Mailers (holds 8 tubes) - 1 box of 8 large mailers + 2 small mailers = $35.00 (includes $5.00 shipping charge)

The Division of Animal Health can supply other materials/items upon request. Please call 609-292-3965 for information and pricing.
